Consolidate non-item object placement

Right now stairs and doors are constants in map_val, store doors and traps are objects. I want to consolidate all non-carryable objects into a features array in the grid.
That means we would have items, monsters, and features possible in each location.
Features would include:
- stairs
- doors
- store doors
- traps
- rubble
